本校將於 11月28日（星期五）上午 9:00–11:00，與高雄市鼓山衛生所共同舉辦 「左流右新知，雙疫苗一起打！」 公費疫苗接種活動。

在活動即將到來之際，提醒大家先確認自己是否符合本次 流感疫苗 與 COVID-19疫苗 的公費接種資格，避免當天才發現不符資格、白跑一趟唷！

✔️ 公費施打對象需具中華民國國民身分或為持有居留證之外籍人士（包含外交官員證、國際機構官員證及外國機構官員證）
 

l  50至64歲無高風險慢性病成人

l  55歲以上原住民

l  65歲以上長者

l  醫事及衛生防疫相關人員

l  6個月以上至國小入學前幼兒

l  具有潛在疾病者，包括（19-64歲）高風險慢性病人、BMI≧30者、罕見疾病患者及重大傷病患者

l  孕婦

l  6個月內嬰兒之雙親

l  幼兒園托育人員、托育機構專業人員及居家托育人員（保母）

l  國小、國中、高中、高職、五專一至三年級學生

l  安養、長期照顧（服務）等機構之受照顧者及其所屬工作人員

l  禽畜相關及動物防疫相關人員

符合公費流感疫苗接種資格的同學、師長與同仁們，提醒您「證件帶齊，施打更順利！」
請記得攜帶 健保卡 以及其他相關身分證明文件，如：身分證、居留證、孕婦健康手冊、兒童健康手冊（包含家長同意書）、重大傷病證明（若未註記於健保卡上）、慢性病患病歷或其他佐證文件、原住民身分證明（如戶口名簿）、醫事人員相關執照或證件等

Dear faculty members, colleagues, and students,

Our university will be partnering with the Gushan District Health Center of Kaohsiung City to host the “Double Protection: Get Your Flu and COVID-19 Vaccines Together!” free vaccination event on Friday, November 28, from 9:00 to 11:00 AM.

As the event approaches, we would like to remind everyone to check whether you are eligible for the free influenza and COVID-19 vaccines. This will help you avoid any inconvenience or an unnecessary trip on the event day.

✔️ Eligibility for Free Vaccination

Eligible individuals include R.O.C. nationals as well as foreign nationals holding an ARC (including diplomatic ID, international organization official ID, and foreign institution official ID).

Eligible groups include:

• Adults aged 50–64 without high-risk chronic diseases
• Indigenous people aged 55 and above
• Seniors aged 65 and above
• Medical and public health personnel
• Children aged 6 months to preschool age
• Individuals aged 19–64 with underlying conditions, including high-risk chronic diseases, BMI ≥ 30, rare diseases, or major illnesses
• Pregnant women
• Parents of infants under 6 months old
• Preschool educators, childcare workers in institutions, and home-based childcare providers (nannies)
• Students in elementary school, junior high, senior high, vocational schools, or year 1–3 of five-year colleges
• Residents and staff of nursing homes, long-term care facilities, and similar institutions
• Workers involved in poultry, livestock, or animal epidemic prevention

✔️ Important Reminder — Bring the Necessary Documents!

If you are eligible for the free influenza vaccination, please remember:

“Bring all required documents for a smooth vaccination experience!”

Please prepare your National Health Insurance (NHI) card, along with any other relevant identification or supporting documents, such as:

• National ID card
• Alien Resident Certificate (ARC)
• Maternity health booklet
• Child health booklet (including parent/guardian consent form)
• Proof of major illness (if not listed on the NHI card)
• Medical documents supporting chronic disease status
• Indigenous status documentation (e.g., household registration)
• Medical professional license or certificate, if applicable

Bringing all necessary documents will help ensure a faster and smoother vaccination process, so you won’t miss out on an important layer of protection for your health!
